How do I create a Kprim question?

A Kprim/Kprime question is a type of multiple-choice question that consists of a statement or question followed by four sub-statements. For each sub-statement, the student must indicate whether it is true or false. The student receives one point if all answers are correct, half a point if all but one answer are correct, and otherwise zero points.
